Dear new and returning families, 

As we prepare for the upcoming 2024-2025 school year, the health office would like to share the process for completing the required forms and documentation for Magnus Health. Below is a checklist of items.

On April 1, 2024, you will receive a reminder email from Magnus Health notifying you that you must take action with your online Student Medical Record. NOTE: Beginning on April 1, you can begin to update/upload the required forms and documents for the 2024-2025 school year. However, you have until June 30, 2024, to complete the information. This gives the nurses time to prepare for the upcoming school year.

For All NEW AND RETURNING students:

An Annual Health Check-Up Is Required

Per DoET and Ministry of Health guidelines, your child must have a health check-up before the start of school each year. Students who have not submitted an annual health check form will not be allowed onto campus until it is completed.

Beginning in the 2024-2025 school year, the health office will accept physicals that are completed on or after April 1, 2024. This means that any physicals done on or after April 1, 2024, will be accepted.

Please note that your child’s health check-up results must be signed by your medical provider and uploaded to Magnus Health by June 30, 2024. You can download and print a physical exam form from Magnus Health, or you can download it here: SSIS Student Physical Exam Form. Print the form and take it to your child’s medical provider to complete and sign at your child’s health check-up appointment. 

NOTE: If your child has a potentially life-threatening condition, you also need to complete and sign an action plan each school year for our Health Office to have on file. We currently require the following action plans: Food Allergy and Anaphylaxis, Asthma, Diabetes, and Seizures

Please review the following checklists of the forms and documents you will need to upload to Magnus Health. For returning students, it should take about 10 minutes, for new students it may take about 20 minutes per child.

RETURNING students: Parents must update/complete each section, upload their child’s new health check-up form and, if applicable, emergency action plan forms.

The following items need to be reviewed and updated in Magnus Health by June 30, 2024:

Conditional Questions

  • Update or answer any new questions.

Vital Health Record 

  • The information you entered last year will still be in the system. You will need to review, update it if needed, and save each section after reviewing.
  • Be sure to update cell phone numbers and emails in PowerSchool.

Consent to Treat

  • Read and sign digitally by typing your name.

Insurance Card Upload

  • Upload a new insurance card ONLY if the current plan has expired.

Physical Exam Form

  • Upload a new physical exam form each year by June 30.

Immunizations

  • Submit documentation of ANY NEW VACCINES your child has received in the last year. (All previously submitted vaccines are saved in your child’s account.)

Consent for OTC (over-the-counter medication)

  • Review and sign digitally by typing your name.

Authorization for Medication

  • Answer “yes” or “no” if your child takes prescription medication at school or on field trips.
  • Upload the completed and signed form if you answer “yes.”

Treatment notes/Medical certificates

  • This is where you would answer “yes or no” if you have any additional medical documents to upload. 
  • You can change your answer throughout the year to upload documents following illness/injury/surgery.
     

NEW Students: Parents must update/complete each section, upload their child’s new health check-up form and, if applicable, emergency action plan forms.

The following items need to be reviewed and updated in Magnus Health by June 30th, 2024:

  • Upload a photo of your child’s official immunization record.
  • If you do not have an official immunization record, print the immunization record form, complete it, and have your child’s medical provider sign it.
